COVERAGE FOR EYE EXAMS
The UHC Choice and Choice Plus options pay 100 % of the cost of a routine eye exam by an in-network optometrist or ophthalmologist once per calendar year. A routine eye exam is when you do not have any medical eye problems and your eyes are examined to determine if you need glasses to correct your vision.
A medical eye examination is the examination, treatment, and management of an eye condition or disease such as cataracts, glaucoma, or other sightthreatening diseases or injuries. The cost for these examinations is treated like other medical examinations by a medical specialist.
SPECIALTY PROGRAM: NAVIGUARD
Participants in our UHC Choice and Choice Plus medical options will be able to benefit from a specialty program.
Naviguard is a program to help you navigate the coverage and cost issues that can arise when you use providers who are not in the UHC network. It combines:
• Robust advocacy services on your behalf, provided by UHC professionals
• UHC’ s extensive and proprietary knowledge of pricing data in the marketplace, and
• Enforcement of provisions protecting patients from egregious billing practices.
If out-of-network medical providers file claims with UHC for services you received, the Naviguard team may contact you to offer their services and reduce your potential out-of-pocket cost.
